A division of Community Access to Coordinated Healthcare (CATCH, Inc.)

This project integrates rural health and local/regional resources to provide care coordination and outreach to rural populations with intense and/or complex health needs. 

The overall goal is to develop a coordination of care model, blending the resources of rural public health and primary care organizations. This will provide essential approaches to care, creating a more integrated healthcare delivery system in the CATCH service area. The project targets populations with health and social needs that impact chronic illness diagnosis and management; or creates a significant risk for adverse chronic health determinants and conditions.
