CATCH serves 26 rural counties across Nebraska through a network of five member organizations: Public Health Solutions, Three Rivers Public Health Department, North Central District Health Department, Southeast District Health Department, and Blue Valley Community Action Partnership.

CATCH is a past grant recipient of Every Woman matters, the AVon Foundation and the komen foundation, using these funds to develop partnerships, increase awareness, provide education on breast health care and offer mammogram services to low income and un/underinsured women. other funding came from the robert wood johnson foundation, kids connections, phhs funding for diabetes and the phone program. Currently, CATCH focuses on implementing rural Health network program improvements and strengthening clinical care coordination with public health efforts for behavioral health and chronic disease management.
